At Hearth and Haven Care we believe every person deserves to feel safe supported and at home. We provide experienced, culturally safe Support Coordination under the NDIS and My Aged Care.
We support people in cities, country towns, and very remote communities, with a strong focus on walking alongside First Nations families and people with complex needs.
Hearth and Haven is founded by Natalie Boyce an award-winning innovator in NDIS and Aged Care and an experienced Support Coordinator. She was the brainchild behind a successful community care service in outback Normanton, which was recognised with the 2025 NAIDOC Community Service of the Year Award. Natalie is now focused on direct case management, bringing her strategic know-how and heart for community into one-to-one support.
She is now bringing that same grassroots leadership to Hearth and Haven Care, where she is now focused on her passion: breaking down barriers and bringing complex Participants the Support Coordination outcomes they deserve, through her NDIS and MyAGedCare knowledge, respect and compassion, and walking beside participants every step of the way.
